Fiscal Year End 8-31-2013, Summary of Services provided by Parish Outreach:

Parish Outreach provided 10,374 services to 1783 persons. These services included: Economic Assistance, Food, Healthcare and Advocacy information and referrals, Housing, Transportation, Employment, Substance Abuse, Case Management/support groups, legal referrals, clothing and furniture assistance and more!

 

Services provided by the Parish Outreach include:

COMMODITY SUPPLEMENTAL FOOD PROGRAM (CSFP)

Each month, in Nassau and Suffolk Counties, INCOME ELIGIBLE seniors, 6 months post partum women, and children who are 5 years old up to their 6th birthday can receive a free supplemental food package.  You must be registered for this program and proper documentation is required.

COMPASSIONATE CARE

Our objective is to bring the love and concern of our Parish community into the homes of our lonely, elderly or disabled neighbors. Volunteers may chat, listen, pray laugh, encourage or simply sit quietly.  Visits are friendly, confidential, long or short, frequent or not, and may or may not include prayer. As Christians we believe that it is part of human nature to reach out, to our God and to one another, in order to help one another.

CONSOLATION MINISTRY

The Ministry of Consolation brings the love and support of our Parish community to families during the time immediately following the death of a loved one. This ministry meets with the family to assist them with planning the Liturgy of Christian burial. Volunteers deliver home baked cakes to the home of the bereaved family, they work on the booklets used during the funeral Mass, act as greeters and attend the Mass as representatives of the Parish  and also serve as Lectors for the Mass when needed.

TELEPHONE REASSURANCE

Parish Outreach offers the Ministry of Reassurance to anyone who is homebound or in a nursing home.  If you, or someone you know, would like a friendly phone call or note, please call our office at 631-261-1695.  Feel free to leave a message and we will return your call.

TRANSPORTATION

 FREE VAN SERVICE IS AVAILABLE TO SENIORS and the mentally or physically challenged without available transportation. We do have a wheelchair lift.
Van trips include the Huntington Senior Nutrition Center, Shopping at Stop & Shop on Wednesday, Shopping at King Kullen on Fridays and to Food Pantry on Pantry Days. (Call P.O. for schedule).
The van is available for Doctor’s appointments and to the Northport V.A.  The van runs MON- FRI 8:30 A.M. – 3:30 PM
The van is available for Sunday 10:30 a.m. Mass.
If interested call Parish Outreach.  Volunteers must do a home visit and intake before van rider approval.
Donations gladly accepted toward gas and maintenance for the van.
